  • This application is designed to help us determine if the adoption is in the cat’s best interest, and to assist you in finding a compatible pet for your lifestyle. An unwise adoption can result in an unpleasant experience for both the adoptive family and the pet and may ruin the pet for future adoption. We hope you agree that the pet’s welfare MUST BE OUR FOREMOST CONCERN. We reserve the right to examine the adoptive situation and decide what is best for the pet overall quality of life. Our goal is to ensure a forever home for each pet that is adopted and a conducive life for the pet involved. I understand these goals and agree to them. I also understand that misrepresentation or omission of facts is cause for denial of adoption. I understand the above questions and I authorize investigation of all statements contained in the application. By signing this application, I am stating that the above mentioned is true.
  • Ringworm is a disease of the skin which can be transmitted to other pets and people. It is treatable with Medical care. This cat may have been housed with cats that tested positive for Ringworm. Cats are not treated for Ringworm upon intake unless there are visual signs present. No signs of Ringworm are present, in our shelter, at this time. When ringworm is present, affected cats are tested and we have results in about 21 days. Medical Waiver By initialing below, you understand that this animal may have been exposed, unawares, to Ringworm and that you will assume all risk associated with allowing this cat to come into contact with any animals or people. I further understand that the Portage Animal Protective League will be unable to provide further assistance with the diagnosis and management of this condition.
  • By initialing below, you agree that you are aware that Feline Upper Respiratory Infection (URI) is similar to a common cold in human beings. The staff of Portage APL has worked very hard to ensure the health of your cat, but URI is especially common in cats that live in a group setting, such as an animal shelter. Incubation is 2 to 14 days and stress can be a major factor. If this cat or any other cat contracts a URI, you agree that you are responsible for proper medical/veterinary care. I will not hold Portage APL responsible for treatment costs and/or spread of such illness. You agree that you are fully aware of the possibility of such illness.
